Efrat Shadmi is an Associate Professor at the University of Haifa, Israel. She received her doctoral degree from the Department of Health Policy and Management, the Johns Hopkins University Bloomberg School of Public Health, Maryland, USA. She has developed the Comprehensive Care for Multimorbid Adults Program (CC-MAP) to provide better self-management support and care coordination for high-cost high-need patients with a nurse – primary care physician based intervention. Her research focuses on methods to better understand the distribution of morbidity and multimorbidity among population groups, developing and evaluating interventions for improving integration of care in multimorbid populations, and on equitable health care delivery and its outcomes. Prof. Shadmi serves as the Co-editor in chief of the International Journal for Equity in Health and is a senior policy consultant to the Department of Health Policy Planning at the largest Israeli non-for-profit health care organization (Clalit Health Services).
